NEW PATIENT INFORMATION

Since 1975, we have been helping patients of all ages feel better, look great and enjoy eating their favorite foods again.  This website is designed to get you acquainted with us, so that you can get the most from every visit. 

On your first visit, we’ll ask you for some health and personal information that will help us complete insurance forms and other correspondence.  Your health information also provides us with some basic history and a place to begin providing you with the kind of dental care you want. 

Previous dental or medical records can be helpful, as can recent x-rays, so please bring these with you or arrange to have them sent to our office.  You can be assured that all such information is held in the strictest confidence.

Following your initial exam, we’ll work with you to develop a treatment program – a “plan of care” – which we believe will bring your mouth to optimal health.  We involve you in this process, discussing our specific findings and taking the time to answer any questions you may have.   Whenever we think it would benefit you, we consult with specialist. 

Because your health, safety and peace of mind are our first priority, our office exceeds beyond all state and federal guidelines for infection control.

As an added service to you, we offer interest free financing.  Payments may also be made with major credit cards.  We will be happy to complete your insurance claim forms and file them with your insurance company.

Appointments may be scheduled Monday through Thursday by calling our office during business hours.  Please remember: appointments are reserved especially for you, and are precisely scheduled to keep your time in our office to a minimum.  Your promptness allows us to extend that courtesy and assurance to all our patients.  Should an appointment change be necessary, we request 48 hours advance notice.
